Hazel Lee Cattenhead, the 2nd of 4 children, was born on October 14, 1942 to Roosevelt and Farrow Clark. Hazel was born in East Chicago, Indiana where she attended Lincoln and Riley Elementary Schools. She graduated in 1962 from East Chicago Washington High School. 

Hazel accepted Christ into her life and became a Christian at an early age. Throughout her life she was a member of several local churches in the Northwest Indiana area.

Hazel had a diverse work history which includes occupations such as a Nurse Aid in Saint Catherine's Hospital in East Chicago, Indiana and at Saint Margaret's Hospital in Hammond, Indiana. She also worked as a Home Health Care Assistant and in other healthcare positions during her working career. She worked part-time positions in-home health care near the end of her career and never formally retired.

Hazel lived in East Chicago throughout her child and early adulthood. She was married four times but separated from her recent husband Emerson Thomas.  Although Hazel did not have biological children, she was a stepmother, "favorite" aunt, and godmother to many who will cherish her memories. 

Hazel's former husband the late Charles Cattenhead assisted with the care, love, and support of her mother Farrow Clark. The respite was needed for her sister Freedonia, who had taken care of her mother for many years. Hazel and Freedonia, two selfless caregivers, managed and coordinated the home care of their mother until she passed away in 1984.

Hazel loved spending quality time with her family and friends. She enjoyed playing cards, dancing, and singing. However, she knew the difference between life's pleasures and it's true treasures. Hazel was full of love and laughter, but most importantly she always worked hard and never neglected her Christian beliefs.

Prior to Hazel's recent return to Northwest Indiana, she resided in Indianapolis along with her sister Freedonia Dent. This move made it possible for Shawn to provide closer medical supervision, transportation and necessary assistance for both of them. 

As Hazel's health continued to decline, she could no longer manage her basic life activities. She was blessed to receive in-home care from her in-laws Eddie and Shirley Bass. Together they provided her with quality in-home health care, transportation, and assistance with everyday life activities. 
However after a long history of medical challenges, Hazel departed life on the morning of July 17, 2018. 

Hazel was preceded in death by her father, Roosevelt Clark Sr; mother, Farrow Clark; and sister, Marlene Doyle. Hazel was also preceded in death within 5 days of her beloved sister, Freedonia Dent on July 12, 2018.

Hazel is survived by her brother, Roosevelt Clark, her nephews Shawn (TaWanda) Dent, Troy Clark, Travis (Michelle) Clark, Coi Clark, Damon Hines, several grand nieces and nephews along with a host of relatives and friends.
